How to replace a lost or damaged UAE-issued birth certificate

Losing a child’s birth certificate or having a damaged one that was issued in the United Arab Emirates (UAE) can be stressful, but replacing it in the UAE is a straightforward process that you can even complete online in 24 hours if all application conditions are met.
Whether the lost or damaged birth certificate belongs to you or your child, the UAE’s Ministry of Health and Prevention (MOHAP) has a streamlined process for applying for a UAE-issued birth certificate.
There are two ways to apply for and obtain a replacement UAE-issued birth certificate:
- Apply at a MOHAP public health centre anywhere in the UAE
- Apply online via the MOHAP website
Step-by-step guide for the issue of a UAE-issued replacement birth certificate and having it delivered to your home or collected from a MOHAP public health centre in the UAE:
Required documents
Before starting the application process, make sure you have the following documents ready:
- A copy of the lost birth certificate, if available
- Passport copies of both parents
- Family Book copy or UAE citizens
If you do not have a copy of the lost or damaged birth certificate, contact the local health authority in the emirate where the child was born to obtain the birth certificate details.
Important requirements
- Both parents or an authorised representative must be present for the application process.
- All documents must be original, including the parents and the applicant’s passports.
- If applying from abroad, provide copies of the original documents and a power of attorney for a representative to receive the birth certificate. Alternatively, for expatriates, a consulate staff member can collect the birth certificate with an authorisation letter stamped by the embassy.
- The status of the original birth certificate will be reviewed before issuing a replacement.
Cost
The fee for replacing a lost birth certificate is AED65.

Step-by-step guide for the issue of a replacement UAE-issued birth certificate online through the MOHAP website:
Visit the MOHAP website
Go to MOHAP’s official website and click on ‘Start Service’.
Log In
Log in using your UAE Pass. If you do not have one, you must create an account with MOHAP.
Select the service
After logging in, find the ‘Birth Certificates’ section on the left-hand menu. Select ‘Issue of a Replacement for Lost Birth Certificate.’
Enter details
Enter the Qaid number (birth notification number) and the date of birth of either parent to search for the birth certificate in the MOHAP system. The Qaid number can be found in the electronic birth notification you received from the hospital.
Verify information
If the record is found, MOHAP will display details such as:
- Baby’s name
- Father’s name
- Mother’s name
- Qaid number
- Date of birth
- Status (e.g., ‘certificate issued’)
- Hospital name
Confirm that the information is correct. Click the three dots on the right-hand side of the screen, then select ‘Add.’ Click the three dots again and choose ‘Edit’ to proceed to the Birth Certificate Lost/Replacement Request page.
Upload documents
Upload a copy of the parent’s passport and the lost birth certificate (if available).
Submit request
After uploading the documents, click ‘Submit.’ You will be directed to the ‘Request Certificate’ service page, where you can specify the number of copies you want in English or Arabic.
Choose delivery option
Select whether you want to collect the birth certificate from a MOHAP Public Health Centre in the UAE or have it delivered to your home address.
Payment
Click ‘Proceed to Payment’ and pay the AED65 fee electronically using a debit or credit card.
Processing
After payment, the MOHAP will review your application. Once processed, the replacement birth certificate will be issued.
If you submit all the necessary documentation and pay the prescribed fee, you can easily replace a lost or damaged UAE-issued birth certificate.
